Job description

Position Title: Registered Nurse – Triage/Telephone
Facility: Monadnock Community Hospital
Location: 452 Old Street Rd, Peterborough, NH 03458
Contract Length: 13 Weeks
Schedule: Monday – Friday, 5x8-hour shifts (40 hours/week)
Pay Rate: $65/hr
License Required: New Hampshire RN License (or Compact) – Must be active at time of submission
Certification Required: BLS (AHA only) – Must be active at time of submission
Weekend Requirements: Occasional Saturdays
On-Call Requirements: None
 
Position Summary:
The Pediatrics Office Nurse (RN) plays a key role in providing and coordinating professional nursing care within a clinical setting. This position emphasizes telephone triage, with approximately 80% of time spent on the phone with patients across all age groups—pediatric experience is required.
Working under medical direction and within the framework of hospital policies and nursing standards, the RN utilizes the Nursing Process (assessment, planning, implementation, and evaluation) to ensure high-quality, patient-centered care. The RN also serves as a resource and mentor to other clinical staff, including LPNs and Medical Assistants.
 
Key Responsibilities:
Telephone Triage: Conduct in-depth patient assessments via phone, provide appropriate care advice, triage urgency levels, and guide patients accordingly.
Direct Patient Care: Provide professional nursing services, including assessments, planning, intervention, and evaluation.
Team Collaboration: Work collaboratively with physicians, fellow nurses, and healthcare staff to create individualized care plans.
Patient Education: Communicate effectively with patients and families regarding diagnoses, treatments, medications, and preventive care.
Medication Administration: Administer prescribed treatments in compliance with safety protocols and clinical guidelines.
Clinical Documentation: Maintain accurate, up-to-date patient records and report any changes to the care team promptly.
Supervision: Guide and delegate tasks to clinical support staff to ensure effective implementation of care plans.
Compliance & Safety: Follow all hospital policies, procedures, and regulatory standards to ensure safe patient care.
Continuous Learning: Engage in professional development to stay informed about current best practices and medical advancements.
Additional Duties: Perform other related duties as assigned.
 
Required Experience:
Recent experience in telephone/triage nursing
Clinical experience with patients of all age groups, especially pediatrics
Ability to clearly communicate with patients regarding medications, medical history, and diagnoses
Preferred: Experience in a Critical Access Hospital, clinic, or medical office setting
