LPN Job Responsibilities:

Bowel Program

Recognizes and reports changes in the patient's condition to the RN supervisor.

Completes timely and accurate clinical notes including addressing patient's progress.

Assists the RN or physician in performing specialized procedures and duties.

Provide client care according to plan of care

Prepares and maintains clinical documentation.

Implements infection control and safety precautions.

Attend team meetings, training, and assists teammates as needed.

Home Health LPN Job Requirements:

Reliable Transportation

CPR Card (BLS)

TB Test (current)

Health Screening

Must pass criminal background check, abuse background check and drug screening.

Current LPN licensed in the state of Illinois (must be in good standing)
