Job description

The Direct Support Supervisor oversees clients and staff in 3 habilitation waiver homes. Duties range from working directly with clients and staff to writing assessments, service plans, and other documentation Responsible for hiring and supervising staff as assigned. Rotates on-call responsibility as assigned to ensure appropriate support and coverage is provided. Provides habilitation and other services to assigned clients within their home and community settings in accordance with the client's service plan This role reports to the Supported Community Living Manager.

What You'll Do:

  • Facilitating admission and discharge procedures for assigned clients.
  • Write annual assessments, service plans, daily documentation and other client documentation for assigned clients.
  • Ensure documentation is accurate and meets billing criteria for all staff under your supervision
  • Participate in hiring, supervision and evaluation of assigned direct support staff.
  • Provide training and job-specific orientation to staff; participates in monthly All Staff meetings.
  • Rotate on-call responsibility as assigned to ensure adequate support and coverage are provided.
  • Communicate regularly with all staff concerning client needs.
  • Ensure clients are scheduled for medical and psychiatric appointments and provide/arrange for transportation as needed.
  • Report personnel issues to the Manager and intervene as directed.
  • Manage Medicaid and other benefits as well as payment tiers for assigned caseload
  • Manage client funds and petty cash for those homes/clients assigned on VF property.

What You'll Need:

  • Associates degree in a human services field required, Bachelor's preferred.
  • Previous experience working with persons with mental illness and/or co-occurring substance use disorders required.
  • A valid driver's license with a driving record
  • Employees are required to maintain a minimum personal vehicle coverage that equates to $100,000 each person and $300,000 each accident.
  • Ability to pass a pre-employment physical with a drug and alcohol screen
