Position Summary
Practice medicine the way you always intended.
At Centerstone, our Primary Care Physicians provide whole-person, integrated care - supporting patients with medical, behavioral health, and substance use needs in a setting that prioritizes time, connection, and outcomes over volume.
This role offers a rare combination of predictable weekday scheduling, mission-driven work in an underserved community, strong interdisciplinary support (behavioral health, care coordination, case management), and meaningful impact on patients who truly need access to care.
You'll deliver comprehensive primary care while collaborating closely with behavioral health teams - allowing you to treat the whole patient, not just symptoms.
This is more than a traditional primary care role. Here, you'll treat complex patients in a high-impact, underserved community, work at the intersection of primary care and behavioral health, and practice in a setting designed to reduce burnout - not create it. This role is ideal for physicians who are seeking purpose-driven work, want to step away from high-volume systems, and value time with patients and interdisciplinary collaboration.
Key Responsibilities
  • Provide comprehensive primary care, including medical evaluations, diagnosis, and treatment planning.

  • Manage chronic conditions and support patients with co-occurring behavioral health and substance use needs.

  • Ensure continuity of care through follow-up and coordination with the care team.

  • Work closely with behavioral health providers, care managers, and support staff.

  • Participate in case reviews and care planning discussions.

  • Support integrated treatment approaches for complex patients.

  • Engage in patient and community health education initiatives.

  • Contribute to program development, quality improvement, and care model advancement.

  • Option to participate in training, mentorship, or supervision of advanced practice providers.

What We're Looking For
Required Qualifications
  • MD or DO from an accredited institution.

  • Board Certified (or Board Eligible) in Family Medicine or Internal Medicine.

  • Active (or eligible) Missouri or Illinois medical license.

  • DEA license.

Preferred Qualifications
  • Experience or interest in behavioral health or addiction medicine.

  • Addiction Medicine certification or willingness to obtain.
