Swift Peace Physical Therapy is seeking a dedicated and compassionate Full-Time Physical Therapist to join our team in West McLean, Virginia. This role is onsite and primarily involves providing physical therapy services in patients' homes, making a significant impact on their recovery in a familiar and comfortable environment. You will operate within a dynamic team of healthcare professionals, including physical therapy assistants, occupational therapists, and nursing staff, collaborating to deliver holistic care and support.

• Conduct thorough evaluations of patients’ physical statuses, ability to perform daily activities, and overall health to develop individual treatment plans in accordance with physician directives.
• Set clear, measurable goals for patient rehabilitation and implement appropriate evidence-based treatment interventions to improve mobility and function.
• Maintain detailed documentation of patient progress, including evaluations, treatment plans, and discharge summaries, to ensure compliance with industry regulations and facilitate optimal care.
• Utilize specialized therapy equipment and modalities to enhance recovery, keeping abreast of advances in physical therapy techniques.
• Supervise and mentor physical therapy assistants and support staff during patient care to ensure effective treatment delivery and nurture their professional development.
• Interact regularly with multidisciplinary team members, including physicians, occupational therapists, and nursing staff, to discuss patient progress and align on care strategies.
• Engage in continuous professional development and training to enhance your expertise and keep current with best practices in physical therapy.

The primary patient population you will serve includes individuals recovering from surgical procedures, injuries, or chronic conditions affecting their mobility and physical function. This diverse group requires tailored treatment plans that cater not only to their physical needs but also to their personal lifestyle, thereby maximizing their ability to regain independence and improve their quality of life.

• Hold a Bachelor’s Degree in Physical Therapy from an accredited institution, ensuring a foundational understanding of rehabilitation principles.
• Possess an active Physical Therapist license in the State of Virginia, meeting all necessary legal and professional criteria for practice.
• Excellent written and verbal communication skills to effectively convey complex information to patients, families, and multidisciplinary teams.
• Proficient in using electronic health record (EHR) systems and other office software to maintain accurate patient documentation.

• A minimum of one year of experience in a physical therapy setting is preferred, providing you with a solid background in outpatient or home care environments.
• Experience with treatment protocols involving various patient populations, from geriatrics to sports rehabilitation, is highly advantageous.
