Our Sayreville outpatient clinic operates in a warm, supportive setting where collaboration among healthcare professionals is encouraged. Key Responsibilities
  • Conduct thorough evaluations of patients, including reviewing medical histories and performing specific assessments to establish treatment plans tailored to each individual.
  • Communicate evaluation findings and treatment strategies clearly to patients and their families, ensuring their involvement in setting recovery goals.
  • Implement tailored treatment protocols based on clinical evaluations, using evidence-based methods to enhance patient outcomes.
  • Regularly consult with multidisciplinary healthcare teams to ensure comprehensive care. Document any changes in patient conditions and modify treatment plans as needed.
  • Prepare and execute appropriate discharge planning, ensuring coordinated follow-up care and maintaining accurate patient documentation, including treatment notes and progress summaries.
  • Manage your time effectively to meet productivity expectations while participating in clinic-wide marketing initiatives to enhance patient engagement.
  • Contribute to the advancement of departmental goals and compliance with healthcare regulations, thereby supporting Swift Health's commitment to superior patient care.

Patient Population / Client Focus

You will care for a diverse patient population, including those recovering from surgery, injuries, or managing chronic conditions. Swift Health Physical Therapy aims to empower our patients through comprehensive care strategies that may involve pain management, rehabilitation exercises, and education on injury prevention, all tailored to their unique circumstances.

Qualifications
  • Bachelor’s degree in Physical Therapy or a Doctorate in Physical Therapy (DPT) from an accredited program.
  • A valid state license to practice physical therapy in New Jersey.
  • Current BLS certification from the American Heart Association.
Preferred Experience
  • At least two years of experience in an outpatient physical therapy setting is preferred, although new graduates are encouraged to apply.
