You will work directly in patients' homes, a critical and intimate setting that promotes personalized care and stronger patient-provider relationships. • Conduct comprehensive evaluations of patients, incorporating medical history reviews and physical assessments to formulate effective treatment plans.
• Clearly communicate evaluation findings and collaboratively set treatment goals with patients and their families, ensuring active involvement in their recovery process.
• Implement individualized treatment protocols based on detailed evaluations, continuously adapting therapy methods in response to patient progress and emerging healthcare practices.
• Engage in interdisciplinary consultations as needed, recognizing significant changes in patient conditions that may warrant adjustments to their care plans.
• Carry out thorough discharge planning and follow-up care, meticulously documenting all aspects of patient interactions, including initial evaluations, treatment progress notes, and discharge summaries.
• Demonstrate effective time management skills, adhering to productivity requirements and actively participating in marketing initiatives that promote our home care services.
• Contribute to the development of departmental goals and participate in compliance with relevant regulatory standards.
• Pursue continuing education opportunities actively to enhance professional competencies, and mentor students as a clinical instructor.
• Provide excellent customer service by ensuring accessibility and flexibility to meet patient needs.

As a Physical Therapist with Swift Health, you will primarily serve a diverse range of patients requiring rehabilitation services in their homes. This includes elderly patients recovering from surgeries or managing chronic conditions, as well as individuals of all ages undergoing rehabilitation for injury recovery. Our holistic approach emphasizes patient-centered care, encouraging a supportive and respectful environment for all clients.

• Bachelor's degree in Physical Therapy required.
• A valid state license to practice as a Physical Therapist in Florida.
• Current CPR certification through the American Heart Association (BLS for Healthcare Providers).
• Strong communication and interpersonal skills.

• A minimum of 2 years of experience in physical therapy, preferably with home care or outpatient settings.
• Experience in patient evaluation and treatment with a focus on rehabilitation.

Swift Health Physical Therapy offers a competitive salary range of $100,000 to $120,000 annually, based on experience and qualifications. In addition to a robust compensation package, we provide a comprehensive benefits program that includes:
