Description
PRIMARY RESPONSIBILITY:
Responsible for coordinating the day-to-day operations of the Empowered Voices Vocational Rehabilitation Program, including Community-Based Skill Building, Community Living Supports, Volunteer Services, participant intake, scheduling, transportation coordination, staff supervision, and program quality to ensure compliance with Federal, State of Michigan, CARF, and agency standards.
ESSENTIAL JOB FUNCTIONS:
  • Coordinates the daily delivery of SVRC's Empowered Voices Vocational Rehabilitation Program in accordance with Federal, State of Michigan, CARF, DOL, and agency standards under the direction of the Chief Clinical Officer.
  • Serves as participant referral and intake point of contact for support coordinating partner agencies; completes paperwork to enroll participants into program.
  • Communicates regularly with referral and participant contacts (partner agency supports coordinators, parents, and/or guardians).
  • Serves as a program point of contact and active participant with Saginaw County Community Mental Health Authority to coordinate services for mutual participants.
  • Seeks out new program referrals to grow program.
  • Assigns case manager for participants accepted into program.
  • Develops and adjusts participant pick-up and drop-off transportation routes based on most current program participant roster, with safety and efficiency as main goals.
  • Develops daily and monthly activity schedules for all program groups; modifies as required.
  • Completes all program and service reports assigned by supervisor.
  • Completes Service Activity Logs (SALs) for participants participating under CCHBC grant.
  • Assists Chief Clinical Officer in ensuring progress notes are completed by community job coaches and entered into the electronic record for each participant daily.
  • Communicates regularly with partner and program activity location points of contact; addresses concerns promptly.
  • Works collaboratively with SVRC's community job coaches, case managers, and employment specialist to ensure excellent program delivery.
  • Monitors program quality, identifies operational issues, and recommends improvements to enhance participant outcomes and program efficiency.
  • Collects data for performance measurement, including but not limited to, distributing and collecting surveys, and entering and analyzing data from participant electronic record.
  • Provides group coverage for community job coaches due to planned or unplanned job coach absence; coverage includes transportation of participants and activity accompaniment and coaching.
  • Ensures that appropriate safety standards and procedures are adhered to in all on-site and off-site program areas; gives input to Safety Committee.
  • Supervises part-time community job coaches.

MARGINAL JOB FUNCTIONS:
  • Attends staff and community meetings, and promotional, trade, and community outreach events as a representative of the Empowered Voices program, as required.
  • Assists Chief Clinical Officer in the creation and delivery of staff training.

QUALIFICATIONS:
  • A bachelor's degree, preferably in a human services field.
  • Supervision experience.
  • Demonstrated ability to organize multiple priorities, solve problems, and exercise sound judgment in a fast-paced environment.
  • Experience working with persons with developmental disabilities.
  • Ability to be CPR and First Aid certified within 90 days of hire.
  • Possession of, or ability to obtain within 90 days of hire, a valid Michigan Chauffeur License and continued eligibility to operate agency vehicles in accordance with SVRC's driver qualification policy.
  • Effective written and oral communication skills.
  • Demonstrated proficiency using Microsoft Office applications, electronic health record systems, scheduling software, email, and other technology required to perform the essential functions of the position.
  • Successful completion of Criminal History Background Check is required under the terms of agency employment.
  • Organized and self-motivated.
  • Able to work as part of a team.
