Position Overview:
The Nurse Navigator is a professional registered nurse (RN) who provides individualized support to patients, families, and caregivers to help overcome barriers within the healthcare system. This role is instrumental in enhancing the patient experience and improving clinical outcomes by facilitating a seamless navigation process across the continuum of care.
The RN is responsible for collaborating with physicians and interdisciplinary teams to triage, coordinate, and manage patient care consistently. They serve as an essential point of contact for patients and families, offering education, advocacy, and emotional support.
The Nurse Navigator participates in strategic initiatives within the clinical program or service line, including the development, implementation, and evaluation of navigation activities for long-term sustainability. This role fosters shared decision-making and patient engagement by forming trusting relationships, addressing communication and health literacy needs, and supporting patients in incorporating their values and preferences into treatment decisions.
The ideal candidate will have previous experience in oncology, nurse navigation, or case management, and will demonstrate a commitment to advancing positive outcomes for patients, providers, and the Sutter Health system.

Graduate of an accredited school of nursing.
CERTIFICATION & LICENSURE:
RN-Registered Nurse of California
BLS-Basic Life Support Healthcare Provider
TYPICAL EXPERIENCE:
5 years recent relevant experience.
SKILLS AND KNOWLEDGE:
Knowledge of medical terminology required.
Ability to ensure follow through with responsibilities and communicate with a varied level of health care staff.
Knowledge of third-party reimbursement helpful.
Verbal and written communication skills.
Microsoft Office software experience required, including Excel and Word
