Mercy Haven is seeking a Supportive Housing Case Manager (Housing Specialist) to provide direct, community-based case management services to individuals and family's needs living in mental health and homeless supportive housing. This role focuses on helping residents obtain and maintain stable housing, build independent living skills, and access critical community resources.
POSITION RESPONSIBILITIES
  • Provide individualized case management services to tenants in supportive housing, including assessment, service planning, and ongoing support
  • Conduct regular home visits and phone check-ins to monitor housing stability, safety, and overall well-being and provides appropriate documentation of all contact
  • Support residents in obtaining and maintaining entitlements and benefits (SSI/SSDI, Public Assistance, SNAP, Medicaid, etc.)
  • Provide crisis intervention and stabilization support as needed
  • Connect tenants to mental health, medical, vocational, and community-based services
  • Support development of independent living skills (ADLs) such as budgeting, cooking, hygiene, cleaning, and use of public transportation
  • Collaborate with service providers and treatment teams to ensure coordinated care
  • Ensures that room/apartments are clean for new tenants (may include cleaning, moving and setting up) & assists tenants with move in and move outs
  • Monitor apartment conditions and report maintenance needs to ensure safe living environments
  • Assists in the transportation of tenants to Mercy Haven programs and events
  • Responsible for Agency compliance requirements by grants through site visits, data collection, and tracking system
  • Participate in supervision, team meetings, and trainings
  • Acts as task supervisor when interns are assigned to Supportive Housing Program
  • Attends and participates in staff meetings and scheduled in-service training and/or additional educational opportunities as well as appropriate agency and interagency meetings
  • Rotation of on-call duties
  • All other relevant duties as assigned

POSITION REQUIREMENTS
  • High School Diploma or Equivalency with experience in human services required
  • Bachelor's Degree in Human Services or related field preferred
  • Experience working with individuals with severe mental illness preferred
  • Valid NYS Driver's License with a safe driving record, access to a personal and reliable vehicle
  • Physical Requirements: Sits, stands, stoops, squats, walks, and moves intermittently during work hours, may push or pull 10-15 pounds daily, may push or pull up to 50 pounds occasionally, drive agency vehicle

*Training takes place Monday - Friday 8:30am - 4:30pm*
SHIFT: Monday-Friday (8:30 am to 4:30 pm), with an on-call availability
PAY RATE: $21.00 per hour
LOCATION: Main Office in Islip Terrace, NY
