the fullest? Vista Care Utah is looking for dedicated, caring, and motivated individuals to
join our team as Independent Contractor caregivers. With different roles available, you can
help these individuals gain independence, access their communities, and thrive in their
daily activities.

Contractor to help someone with an intellectual or developmental disability live their life as
independently as possible, ensure they're part of their community, and ensure they live life
to the fullest.
JOB TYPE: Independent contractor
COMPENSATION RANGE: Dependent on Services Provided
LOCATION: Provo, Utah and surrounding areas
POSITION RESPONSIBILITIES:
As a direct support contractor, your role and responsibilities will vary depending on the
type of service.
Respite Provider:
• Temporarily taking an individual with disabilities into your home, or staying at the
home where the individual resides, while their regular caregiver is unavailable.
• Maintain the individual's health, safety, and wellbeing.
• Assist with activities of daily living and personal care as needed.
• Support recreational, social, and community-based activities.
• Follow the individual's support strategies, behavioral supports, dietary needs, and
other identified care requirements.
• Provide transportation when authorized and appropriate.
• Complete required service documentation and communicate relevant information
to Vista Care and/or the regular caregiver.
• Pay rate is negotiated between the regular caregiver and the respite provider. Typical
pay ranges from $100 per day to $125 per day depending on the individual's funding
level and negotiations between the caregivers.
Supported Living Provider:
• Provide individualized support to help people live as independently as possible in
their own home and community.
• Assist with daily living skills such as cooking, cleaning, laundry, personal care, and
household responsibilities.
• Support budgeting, shopping, money management, and other independent living
skills.
• Assist individuals with accessing community resources, appointments, activities,
and social opportunities.
• Provide transportation and community access as authorized.
• Promote self-determination, choice, communication, advocacy, and development
of independent living skills.
• Support health and safety needs according to the individual's support plan.
• Complete required service documentation and communicate with the individual's
support team.
• Pay begins at $19.00 per hour and may be more based on the individual's needs
level
Day Supports (Individual) Provider:
• Provide individualized daytime support and supervision to people with intellectual
and developmental disabilities.
• Assist individuals in developing a community-based activity calendar with
meaningful community access opportunities.
• Assist individuals in developing and maintaining social, communication, self-help,
and independent community living skills.
• Support participation in meaningful community-based activities based on individual
interests and goals.
• Encourage independence, choice, self-advocacy, and development of natural
community connections.
• Assist with personal care and activities of daily living when needed.
• Provide transportation to community activities and appointments when authorized.
• Implement individualized goals and support strategies.
• Maintain required documentation regarding services, activities, progress, and
significant events.
• Pay begins at $19.00 per hour and may be more based on the individual's needs
level
Requirements
• Must be 21 years of age or older.
• Must have a valid driver's license, reliable transportation, current and adequate
vehicle insurance, and current vehicle registration.
• Must be able to pass a combination of all or some of the following: state and/or
federal background check, OIG check, sex offender registry check, or others as
required by regulation or best practice.
• Must have a reliable computer and internet access or ability to use cellular phone
for documentation and communication purposes.
• Must be able to pass all state-mandated and/or Vista Care-required training courses
(provided free of charge).
