About New Heights Care Connections

New Heights Care Connections (NHCC) is a community-based, technology-supported care coordination company on a mission to close care gaps and remove barriers for people who have been hardest to reach. We work alongside health plans, ACOs, and provider groups to make sure members — especially those dealing with chronic conditions, recent hospitalizations, or unresolved social needs — don’t fall through the cracks.

Our Care Navigators are not just outreach workers. They are the last-mile connection between a system that can feel overwhelming and the people who need it to work. We bring the care to the community.

The Role: What You’ll Actually Do

As a Care Navigator with NHCC, you will work directly in the field, in homes, communities, shelters, and neighborhoods, to support members who are high-risk, hard-to-reach, or recently discharged from the hospital. You’ll be their guide, their navigator, and their advocate.

Day-to-Day Responsibilities

•      Conduct in-home visits and telephonic outreach to engage members who have gone quiet or are hard to reach

•      Complete Health Risk Assessments (HRAs) and Social Determinants of Health (SDoH) screenings to uncover what’s really going on in a member’s life

•      Connect members to food assistance, utility support, housing resources, transportation, and community programs

•      Support members following a hospital or ER discharge, confirm follow-up appointments, medication understanding, and care plan adherence

•      Schedule appointments and coordinate transportation and other practical barriers to care access

•      Provide health education using approved materials and help members understand their benefits and how to use them

•      Identify and escalate urgent clinical, behavioral health, or safety concerns to the care team immediately

•      Document all visits and interactions accurately and on time in our technology platform

•      Participate in team huddles, case reviews, and ongoing training to continuously improve your practice

Who You Are: Minimum Qualifications

Required

•      High school diploma or GED (required)

•      1+ year of experience in community health, social services, healthcare, case management, or a related field OR lived experience navigating healthcare or social systems that you bring to this role

•      Certification as a Care Navigator through the state of Ohio, OR willingness and ability to obtain certification within 6 months of hire

•      Valid driver’s license and reliable personal vehicle (mileage is reimbursed)

•      Comfort working independently in the field with minimal supervision

•      Strong interpersonal skills — you know how to build trust quickly with people from all walks of life

•      Comfort using a smartphone and basic technology for documentation and communication

•      Ability to maintain confidentiality and handle sensitive health information in compliance with HIPAA

•      Ability to pass a background check and federal OIG exclusion screening

Preferred (Not Required)

•      Associate’s or Bachelor’s degree in health, social work, public health, or a related field

•      Bilingual (Spanish or other languages spoken in your community are a strong asset)

•      Prior experience with Medicaid, Medicare Advantage, or value-based care populations

•      Experience conducting in-home visits or working with members post-discharge

•      Familiarity with local community resources in your service area

•      Experience with motivational interviewing or trauma-informed care approaches

Training & Compliance Requirements

All NHCC Care Navigators complete required training before conducting independent visits. This includes:

•      Mandatory reporting and safeguarding requirements
