Overview
Guardian Community Care is hiring dedicated CNAs and PCAs
to provide quality, person-centered care to clients in their homes throughout Wake and Chatham counties. In this role, you'll help clients maintain their independence, dignity, and quality of life while working in a supportive environment that values compassion, professionalism, and teamwork.

Duties & Responsibilities
  • Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, toileting, and mobility
  • Monitor and document client health status, vital signs, and behavioral changes
  • Provide medication reminders according to care plans and HIPAA guidelines
  • Prepare nutritious meals tailored to dietary needs and assist with feeding when needed
  • Maintain a clean, safe environment through housekeeping and infection control practices
  • Safely assist with transfers using equipment such as Hoyer lifts or client-specific mobility devices
  • Provide dementia, Alzheimer’s, and memory care support
  • Communicate effectively with supervising nurses, care coordinators, and family members
  • Complete accurate EVV documentation for all visits
Requirements
  • Current TB Test (required)
  • CNA applicants must have an active NC CNA listing. PCA applicants must have prior personal care or caregiving experience. 
  • Experience with Electronic Visit Verification (EVV) systems
  • Ability to perform physical tasks including lifting, transfers, and mobility assistance
  • Experience supporting individuals with developmental disabilities, autism, dementia, or Alzheimer’s is highly valued
  • Reliable transportation and ability to travel within Wake and Chatham counties
  • Professional scrubs are required for all assignments and or client visits 
  • Must pass a pre-employment drug screen and background check
  • Compassionate attitude and commitment to person-centered care
  • Prior experience in home care, assisted living, nursing homes, or group homes preferred
  • CPR/First Aid certification preferred
  • HIPAA training or familiarity with privacy standards is a plus

Work Location
In-person assignments throughout Wake County and Chatham County, including Raleigh, Garner, Clayton, Cary, Apex, Pittsboro, Siler City, and surrounding communities.
