Suncoast Hospice Rn Case Manager

Suncoast Hospice, a member of Empath Health, is seeking a caring and motivated RN Case Manager to work with us in North Pinellas while providing exceptional care. Coverage needs average 8 shifts per month (64 hours), including one weekend shift each month.

Since 1977, Suncoast Hospice has been honored to serve our neighbors across Pinellas and Hillsborough counties with compassionate end-of-life care. As part of Empath Health, we help those living with advanced illness find comfort, dignity, and meaning, while supporting their families and caregivers with guidance and care every step of the way.

What you'll do

• is responsible for identifying and coordinating patient/family needs and for providing preventative and supportive care to the terminally ill patient/family unit. • utilizes assessment, intervention and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.

What you'll need

  • Associate's degree (A.A.) or equivalent from two-year college or technical school; minimum of one year related experience and/or training; or equivalent combination of education and experience.
  • Valid Florida license as a Registered Nurse
  • Valid Florida state driver's license
  • Prefer one (1) year experience in an acute or post acute care setting.
