At Garden Spot Village, our Skilled Nursing LPNs provide direct and indirect resident care, administer medications, perform treatments, and support Certified Nursing Aides (CNAs). This position ensures high-quality, person-centered care while promoting resident safety, comfort, and dignity.
What You'll Do
  • Comply with established policies, procedures, and standards, including the Compliance Program/Code of Conduct
  • Follow Residents' Rights policies at all times
  • Maintain confidentiality of all resident information, including conditions and family matters
  • Follow established standards of nursing practice and implement facility policies and procedures
  • Communicate pertinent information during and between shifts to ensure continuity of resident care
  • Conduct daily resident rounds and report problems to the nursing supervisor, initiating corrective actions as needed
  • Identify safety hazards and initiate corrective actions immediately
  • Participate in facility education programs
  • Supervise CNAs providing direct resident care and coach them according to established schedules
  • Ensure compliance with federal, state, and local regulations within the assigned unit
  • Build relationships with residents and families to obtain daily preferences and communicate them to CNAs in alignment with Person-Centered Care philosophy
  • Administer and document direct resident care, medications, and treatments per physicians' orders
  • Notify physicians of changes in resident condition, medication errors, abnormal lab/X-ray results, or any unusual observations
  • Participate in development and implementation of resident interdisciplinary care plans
  • Respond promptly to resident calls for assistance
  • Count narcotics, check emergency kits, and manage exchange keys
  • Provide daily instruction to CNAs regarding care and treatment procedures
  • Provide direct resident care as necessary, in addition to supervisory duties
  • Collaborate with residents, nursing staff, physicians, families, consultants, community agencies, and ancillary service providers
  • Participate in meal service while monitoring dietary needs of residents
  • Order, receive, and store medications appropriately
  • Ensure resident safety under your supervision
  • Observe all facility safety policies, including universal precautions for infectious waste and disease
  • Maintain professional appearance and wear a clean, neat uniform
  • Demonstrate dependability and punctuality by coming to work as scheduled
  • Participate in quality assurance activities and relevant in-services/training sessions
  • Engage residents in household activities, documenting participation and responses in PointClickCare
  • Perform other duties as assigned

What We're Looking For
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) in Pennsylvania under the State Board of Nursing
  • Ability to rotate shifts and work weekends as scheduled
  • Strong organizational, initiative, judgment, dependability, and leadership skills
  • Ability to work congenially with others and delegate wisely
  • Ability to teach necessary skills for proper care and documentation
  • Excellent customer service skills and ability to interact professionally with residents, families, physicians, and other visitors
  • May rotate shifts and work Saturdays, Sundays, and holidays as scheduled
  • May be available for evening/weekend activities or assist during weather emergencies

Additional Information
As a FLEX (per diem) employee there is a minimum of 16 weekend hours per month,1 summer holiday and 1 winter holiday per year. No minimum weekday shifts!
