Job description

Position Summary

Under direct supervision of the Primary Care Director, the Referral Specialist provides a centralized source for information regarding referral for services, the referral process, receiving and tracking referrals, and referral sources. This is a 36 hour /week position eligible for our generous benefits package which includes Health, Dental and Vision Coverage, HSA, Flex Spending, Short Term and Long Term Disability, Life Insurance, Generous PTO and paid holidays!

Qualifications

  • A high school diploma or equivalent.
  • Medical terminology and EPIC experience preferred.
  • Bilingual a plus (Spanish/English).

Key Accountabilities

Schedules all referrals placed by PCHC providers to external specialties.

  1. Maintain ongoing, timely tracking and appropriate documentation of referrals. Verify accurate patient demographic and current insurance information. Maintain external referral information, such as facility phone number, address, and contact person.

  2. Assemble information concerning patient's clinical background and referral needs per referral procedure.

  3. Perform scanning of incoming forms such as but limited to reports from specialist, external records and results.
  4. Notify ordering provider of any referral no shows, cancellations or patient refusals.
  5. Meet with patients in rooms to schedule Medical, Behavioral Health and imaging appointments.
  6. Perform daily follow-ups of both the active and deferred work queues to determine referral status.
  7. Participate in quality improvement activities as required. May participate in clinical outreach activities.
  8. Supervise and orient new clinical employees, students and volunteers. If bilingual, assist with translation as indicated.
  9. Provide specialty services, lab and x-ray vouchers for patients as indicated.
  10. Authorize, and complete referral vouchers.
  11. Perform other duties as assigned.
