The Opportunity:

The PSW Coordinator reports to the Director of Care and works collaboratively with the senior management team to plan, organize, direct and coordinate resident personal care services within approved policies and procedures; to promote and support PSWs to provide the best care possible in accordance with AgeCare's mission and values.

What Is In It For You:

  • Assumes full responsibility for the general day to day operations of the Personal Support Workers (PSWs).

  • Participates in the hiring, orientation, evaluation and corrective action of PSWs.

  • Monitors the care and services provided to residents through direct supervision and formal and information audits, on all shifts.

  • Promote improved practices through mentorship, coaching and education.

  • Ensures a high focus on quality within the home as a member of CQI committee, assists mandatory program leads with quality initiatives.

  • Leader in person-centred philosophy, Imagine initiatives and restorative care approaches.

  • Ensures all PSWs mandatory annual education completion.

  • As a Lift Champion, responsible for leading the Lift & Transfer Program ensuring training and competencies of employees and students.

  • Assists the Nursing Team in development and evaluation of personal care programs.

  • Lead monthly PSW & Imagine meetings.

  • Attends resident care conferences as required.

  • Laison with local schools and instructors for student placement.

  • Lead and coordinate the Student Placement Program for the home which includes coordination of preceptors, orientation and onboarding, regular touchpoints with students and preceptors and evaluation of placements.

What You Will Need:

  • Graduate of Personal Support Worker (PSW) Certificate/Diploma Program that meets the standards outlined in the Fixing Long Term Care Homes Act.

  • Must have worked as a Personal Support Worker in Long Term Care.

  • Knowledge of procedures and techniques involved in administering simple treatments and related bedside care to residents.

  • Behavioural Support Ontario (BSO) certification and Imagine training is considered an asset.

  • Strong computer literacy and typing skills, familiarity with programs such as Word, Excel, PCC, PPOC, Workday, UKG, Outlook is an asset.

  • Must understand the basics of good sanitation and clean techniques, Knowledgeable with respect to the needs of geriatric residents.

  • Must be mature with effective communication (both written and verbal) and interpersonal skills.
