Job Description:

Essential Function
SERVICE

  • Responsible for coordinating weekly structural heart case conference.
  • Assess patient health status through physical exam reviews patient history, diagnostic tests andprocedures and review labs and other critical data.
  • Diagnose acute and chronic conditions affecting all organs systems.
  • Initiate orders for medications, laboratory, and other diagnostic testing and medical treatments onappropriate forms.
  • Communicates with courtesy when interacting with internal or external customers - in person or on thetelephone.
  • Demonstrates respect for co-workers and develops positive working relationships with otherdepartments.
  • Incorporates Caring Model behaviors and culturally competent care into daily practice. Demonstrates organizational commitment standards and values.
  • Assists with facilitating an evidence-based, multidisciplinary, multispecialty approach to care throughoutthe care continuum.
  • Collaborates with Physicians, Management, and staff in navigating patients (needs ranging from minimalto complex) from the diagnostic phase through treatment planning, medical management andsurveillance/ survivorship assuring patient have appropriate, timely, medical and supportive services.
  • Completes accurate data collection (per direction of Physician and Management) with all appropriateinformation in a timely and efficient manner.
  • Remains current in evidence-based practice and new emerging trends in area.
  • Develops appropriate relationships to maximize patient experience across departmental, campus and
  • physician/ provider lines.
  • Maintains working knowledge of JC/CMS/AHCA and Core Measures.
  • Works with Case Management to assure appropriate utilization of services and length of stay for cardiacpatients.
  • Write and/or dictate progress notes, procedure notes, history and physical examination,transfers/discharge summaries in accordance with medical policies.
  • Evaluate the processes and outcomes to enhance the delivery of excellent, cost-effective health care.

PEOPLE

  • Adheres to organizational standards for dress and professional appearance.
  • Demonstrate collaboration and teamwork in managing patient flow-i.e. promotes timely patientplacement by following hospital policy for patient admission, discharge and transfer.
  • Participates in peer interviewing and peer review process.
  • May supervise RN, LPN, or other members of care team.

FINANCE

  • Considers factors related to safety, effectiveness, cost and impact on practice in the delivery of nursingservices.
  • Monitor and report out on key financial metrics related to structural heart patients (i.e. volumes, LOS,cost per case, margins etc.)
  • Evaluate opportunities to function more efficiently while improving quality and decreasing cost.
  • Assist with development of strategy to increase volume/market share of the structural heart patientpopulation.

GROWTH:

  • Demonstrates commitment to professional and personal growth-i.e. completes all mandatory educationalrequirements, maintains unit based certification.
