Job description

Position Summary:

Under the direction of the PACE Director of Technology, the PACE Project Management Specialist is responsible for managing and supporting key initiatives that advance clinical quality, operational efficiency, participant safety, and regulatory compliance across the PACE program. This role leads projects that involve interdisciplinary teams, frontline staff, and organizational leadership, ensuring alignment with PACE model requirements and best practices. Core responsibilities include:

  • Project planning and coordination: Define project scope and objectives, gather clinical and operational requirements, establish Key Performance Indicators (KPIs) relevant to PACE quality and participant outcomes, and develop milestone and deliverable timelines in collaboration with interdisciplinary team members
  • Managing project constraints: Oversee time, budget, scope, and quality through structured project and changemanagement methodologies, ensuring projects support compliance with CMS, DHCS, and PACE regulatory standards
  • Risk management: Identify and mitigate risks, operational gaps, and clinical or regulatory impacts throughout the project lifecycle, elevating issues and decision points to leadership as appropriate
  • Team leadership and engagement: Facilitate effective collaboration among clinical, operational, IT, and administrative teams; motivate crossfunctional stakeholders; and ensure clear roles, responsibilities, and accountability to drive project success
  • Stakeholder communication: Provide consistent, transparent communication to leadership, project teams, and frontline staff; develop clear updates and briefing materials; and ensure organizational alignment on project goals, timelines, and outcomes

Essential Functions of the Job:

  • Lead major organizational initiatives as well as multiple mid and smallscale projects that impact clinical care delivery, regulatory compliance, operational efficiency, resource allocation, and participant and staff experience
  • Serve as the primary organizer, facilitator, planner, and problem solver, coordinating resources and managing all aspects of assigned projects with full accountability for execution and outcomes
  • Engage key stakeholders to develop evidencebased recommendations informed by collective input, observations, data, and analysis; provide strong leadership to project teams to ensure objectives are met within established timelines
  • Support, implement, and integrate technology solutionsโ€”including Epic and related systemsโ€”to enhance clinical quality, operational performance, and resourcemanagement strategies
  • Apply riskstratification methodologies to better understand participant populations and inform project design and decisionmaking
  • Collaborate closely with IT, clinical operations, ambulatory and inpatient services, and healthplan teams to ensure alignment, effective communication, and successful project delivery

Additional Duties and Responsibilities:

  • Leads complex, high impact care initiatives and strategic projects across San Diego PACE, including major organizational efforts and multiple midย  and small scale projects
  • Develops, organizes, facilitates, and oversees comprehensive project plans for assigned care initiatives; identifies and mitigates risks, resolves issues, coordinates resource needs, and ensures projects meet established timelines, budget parameters, and quality expectations
  • Partners with executive leadership to define project scope, establish Key Performance Indicators (KPIs), provide regular project status updates, present outcomes, and remove barriers to progress
  • Collaborates with senior leaders, management, physicians, and frontline teams to understand clinical, operational, and business requirements; formulates recommendations based on stakeholder input, observation, data analysis, and best practices; and drives implementation across all San Diego PACE service areas
  • Analyzes operational systems, workflows, and data to identify performance gaps and opportunities for improvement
  • Consistently delivers high quality, stakeholder aligned solutions that support organizational priorities and participant centered care
  • Fosters communication, collaboration, and consensus building among stakeholders and project team members to ensure alignment and shared ownership of project goals
  • Partners with leadership and project teams to ensure standards, technologies, training, and performance measures are developed and implemented to support project success and long term sustainability
  • Advises operational, clinical, and technical leaders on care related initiatives, providing guidance on deliverables, interdependencies, risks, and data insights to support coordination across service lines, departments, and project teams
  • Monitors and reports progress across multiple concurrent projects to ensure milestones, timelines, and deliverables are achieved
  • Serves as a change agent in the planning, implementation, and evaluation of value based care strategies, resource management initiatives, and care model improvements
  • Participates in systemwide collaboration with interdisciplinary teams across San Diego PACE to deepen understanding of the care model, identify improvement opportunities, and apply evidence based best practices
  • Familiarity and ability to use computers as well as EHRs
  • Complies with all department, organization, and government policies & procedures
  • Attends meetings and trainings as required.

Job Requirements


Experience Required:

  • Requires 3 or more years of experience in the health care industry
  • 2 years of demonstrated project management experience

Education Required:

  • Bachelor's degree in healthcare, business administration, or related disciplineย

  • Meet a standardized set of competencies for the specific position description established by San Diego PACE and approved by CMS
