Thrive Compassionate Counseling is seeking a dedicated and licensed Clinical Social Worker (LCSW) to join our team in a rewarding full-time remote position. Work Environment

In this remote role, you will interact closely with an interdisciplinary team of healthcare professionals—including physical therapists, occupational therapists, and speech-language pathologists—working collaboratively to support patients on their journey to recovery. While day-to-day tasks may be conducted from a home office, you will remain integrated with the team through digital platforms, ensuring continuity of care and communication. Key Responsibilities
  • Conduct thorough psychosocial assessments using interviews, observations, and comprehensive medical record reviews to identify the emotional, social, and environmental factors affecting patient recovery.
  • Provide empathetic counseling and emotional support to patients and their families, addressing adjustment challenges and fostering resilience during the rehabilitation process.
  • Collaborate with the rehabilitation team to create personalized treatment plans that encompass patients' psychosocial needs, ensuring a holistic approach to their care.
  • Assist patients in accessing essential community resources, such as support groups, vocational rehabilitation services, or housing assistance, that can aid in their recovery journey.
  • Educate patients and families on community resources and post-rehabilitation services to encourage sustained progress and integration after discharge.
  • Participate actively in discharge planning meetings, facilitating smooth transitions home or into other care environments and ensuring patients are well-prepared for the next steps.
Patient Population / Client Focus

Your work will primarily involve patients recovering from various conditions, including stroke, spinal cord injuries, orthopedic surgeries, and neurological disorders. This role is pivotal in bridging the gap between physical recovery and emotional wellbeing, enabling patients to regain not only their mobility but also their confidence and independence. Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
