A Brief Overview
The Coordinator of the Diabetes Education program is responsible for oversight, planning, implementation, coordination and evaluation of all aspects of this patient education program at all Children's Hospital diabetes education sites.
Essential Functions
- Chairperson of the Diabetes Center Advisory Board. Plan 2 to 4 meetings annually which conform to ADA guidelines. Meetings can be in person or via e-mail. Prepare for and conduct annual total program review which includes: Review of program goals and goal attainment for the previous year, Identify mechanisms by which participants access our program Tabulation of patients who completed the program during the previous year Tabulation of patients receiving follow up education during the previous year Time spent providing initial education(both RN and RD) Review of Mission Statement Review of organizational structure Identification of target population including statistics on type of diabetes, age of patient, and ethnicity, identification of cultural factors impacting our education program and delivery of education Identification of the resources available to the program (personnel, budget, and equipment), discussion of ongoing curriculum review, discussion of community concerns, and review of behavioral (Mutually agreed upon goals) and outcome data (Ale results pre and post education and patient satisfaction scores). Prepare agenda for additional meetings held throughout the year as needed. On-going communication with Advisory Board members as needed
- Maintain and enhance on-going coordination of education program Coordinates and collaborates with appropriate resources (ie: providers, leadership team, content experts, organization development) to analyze needs in order to lead development, implementation, evaluation and maintenance of comprehensive continuing education programs for patients, families, and staff based on holistic age specific patient/family educational needs assessment and annual staff needs assessment, staff request, department direction, variance trends and patient/parent and other customer's satisfaction. Educational activities reflect identified needs of target audience Independently creates programs that incorporate adult learning strategies, ethical principles, critical thinking skills, professional growth, open communication and collaborative relationships. Quality of information developed is professional with regards to organization of information, clarity of instruction, attention to details and presentation style. Work together with other diabetes center team members to continually evaluate and update diabetes program content; outcomes; make recommendations for changes; develop, implement and evaluate goals, etc. (as per ADA guidelines). Communicate with representatives of companies that supply technology and medication used for management of diabetes to obtain the most current info and technological tools (meters, insulin delivery methods, pumps ,sensors, computer programs, education material, etc.) used by patients to achieve optimal diabetes management. Communicate updated diabetes management information to all stakeholders (physicians, in-pt. hospital staff, patients and families) Ensure continuous education program compliance with ADA recognition guidelines for the Diabetes Education program at Children's Hospital. Preparation of ADA annual status reports and recognition extension application every 4 years. Ensure ADA audit readiness at any given time. Evaluate the effectiveness of the education program. Coordinate and participate in development of Community diabetes education programs Coordinate and participate in the development of staff diabetes education programs (in-service). Ensure all diabetes education instructors maintain qualifications (current licensure, CE credits, and registrations). Engage in diabetes education with patient s and families. Support outreach clinics with patient education and teaching materials as needed. Cover inpatient education on a weekend call rotation with other clinic nursing staff.

Education Qualifications
- Bachelor's Degree Bachelor's degree from an accredited college or university in social work, nursing, dietetic, or related field Required and
- Academic or experiential preparation in program management Preferred
Experience Qualifications
- Minimum of 2 years' experience in Diabetes Education or related field including academic or experiential preparation in the care of people with a chronic disease Required
Skills and Abilities
- Knowledge and proficiency in applying the nursing process, nursing skills, and nursing research into patient care, teaching, and systems.
- Ability to communicate effectively both verbally and in writing to include use of computer technology in program presentations and/or data analysis.
- Able to be self-directed, with minimum supervision, and detail oriented.
- Must be able to perform a variety of duties, often changing from one task to another of a different nature, without loss of efficiency or composure, and be able to perform under stressful conditions when confronted with multiple requests, demonstrating prioritization skills.
Licenses and Certifications
- Certified Diabetes educator (CDE) required within 2 years of hire. Required
