As a full-time Social Worker, your expertise will be utilized to provide critical support and advocacy for seniors and their families, fostering their overall well-being and quality of life. This position offers a fully remote work setting within a Detox Center that provides an array of services, including support for mental health and substance-related issues. Our flexible work environment allows Social Workers to engage with clients and families from the comfort of their own homes while fulfilling the obligations of home visits and client assessments. You will work closely with a diverse interdisciplinary team, including healthcare professionals, case managers, and community affiliates, all focused on delivering holistic care to our clients.

• Perform detailed psychosocial assessments of clients to identify emotional, mental, and social needs.
• Develop individualized care plans in collaboration with clients and their families, incorporating each client’s preferences, goals, and resources.
• Provide direct counseling services to seniors, addressing transition challenges such as aging, loss, and healthcare changes.
• Act as a client advocate, helping to navigate and mobilize community resources, ensuring access to necessary services and supports.
• Collaborate with healthcare professionals and caregivers to facilitate comprehensive care approaches and resolve client care discrepancies.
• Conduct regular wellness visits to check on clients’ living situations and reassess changing needs.
• Engage families in open communication to mediate concerns and reinforce care strategies.
• Educate clients about community resources, support avenues, and available programs that can enhance their social engagement.
• Maintain meticulous documentation of client interactions, assessments, and care plans within electronic health record systems to ensure compliance and continuity of care.
• Stay continually informed about changes in elder care practices and regulations to uphold highest ethical standards in social work.

This role primarily focuses on the senior demographic, often dealing with individuals facing unique challenges associated with aging. The social worker's responsibilities will directly impact their quality of life, ensuring they receive holistic, client-centered support that honors their unique experiences and needs.

• A Master's Degree in Social Work from an accredited institution.
• Current, valid state licensure as a Licensed Master Social Worker (LMSW).
• At least two years of experience in social work, preferably in a community-based or geriatric care setting.
• In-depth knowledge of aging-related issues, including the psychosocial factors affecting seniors.
• Strong assessment, counseling, and problem-solving capabilities.
• Ability to demonstrate empathy, compassion, and establish rapport with elderly clients and their families.
• Exceptional communication and interpersonal skills to collaborate effectively within an interdisciplinary team.
• Proficient in the use of electronic health record (EHR) systems and related software applications.

• Experience in a detox or rehabilitation center working with seniors.
• Proven track record of advocating for clients’ rights and needs in complex situations.
• Familiarity with community resources specific to Pembroke Pines and the greater Florida area.
