This position offers a fully remote work setting within a Detox Center that provides an array of services, including support for mental health and substance-related issues. Our flexible work environment allows Social Workers to engage with clients and families from the comfort of their own homes while fulfilling the obligations of home visits and client assessments. You will work closely with a diverse interdisciplinary team, including healthcare professionals, case managers, and community affiliates, all focused on delivering holistic care to our clients.

Key Responsibilities
  • Perform detailed psychosocial assessments of clients to identify emotional, mental, and social needs.
  • Develop individualized care plans in collaboration with clients and their families, incorporating each clientโ€™s preferences, goals, and resources.
  • Provide direct counseling services to seniors, addressing transition challenges such as aging, loss, and healthcare changes.
