Job description

Evergreen post Acute is hiring Registered Nurse (RN) Assistant Director of Nursing (ADON) to join our long-term care team in Smyrna, DE.

Responsibilities:

  • Assist in overseeing daily nursing operations

  • Support preparation and participation in state and federal surveys

  • Monitor QI/QM data and survey reports; provide recommendations for improvement

  • Serve on the Quality Assurance and Assessment Committee

  • Participate in care plan meetings and interdisciplinary collaboration

  • Assist with discharge planning coordination
