Position Summary

The Case Manager Recovery Coach provides home- and community-based case management and recovery support to parents and caregivers impacted by substance use. Working within NNCC’s Safe & Stable Caregivers Substance Use Program, this position helps families achieve recovery and service plan goals, strengthen family stability, access treatment and community resources, and reduce barriers to successful engagement in services.


Essential Duties and Responsibilities

Case Management & Family Support

  • Maintain an assigned caseload and serve as the primary point of contact for participating families.
  • Provide intensive home- and community-based services designed to strengthen families and support successful recovery.
  • Engage participants regularly, including approximately two to three in-person contacts per week or as required by individual service plans and program guidelines.
  • Assess participant strengths, needs, goals, and barriers and participate in the development and implementation of individualized service plans.
  • Support parents and caregivers with routines, budgeting, household management, healthcare access, stress management, and other needs that contribute to family stability.
  • Provide crisis support, advocacy, problem-solving, and service coordination within the scope of the position.
  • Provide transportation or transportation assistance when authorized and necessary to reduce barriers to treatment and services.

Recovery Coaching & Resource Navigation

  • Provide person-centered recovery coaching that supports participants in identifying and pursuing their individual recovery goals.
  • Educate participants about available treatment, recovery supports, and multiple pathways of recovery.
  • Use Motivational Interviewing and other strengths-based approaches to support engagement, self-efficacy, and positive change.
  • Identify barriers to treatment and recovery and coordinate referrals, linkages, and warm handoffs to appropriate providers.
  • Connect participants with substance use treatment, behavioral health services, medical and dental care, public benefits, and other community resources.
  • Maintain current knowledge of substance use treatment, recovery services, and community resources available throughout the Commonwealth.

Collaboration & Service Coordination

  • Collaborate with DCF, substance use treatment providers, behavioral health providers, healthcare professionals, and other community partners involved with participating families.
  • Participate in case conferences, provider meetings, care planning, and other collaborative activities.
  • Maintain regular communication with program leadership regarding participant progress, barriers, safety concerns, and service needs.
  • Promote coordinated, family-centered services that support recovery, child safety, and family well-being.

Documentation & Compliance

  • Maintain accurate, timely, and confidential case records, progress notes, service plans, referrals, and other required documentation.
  • Complete required program reports and data entry according to established deadlines.
  • Utilize Virtual Gateway/OASIS and other required electronic systems in accordance with program procedures.
  • Ensure services and documentation comply with program requirements, agency policies, confidentiality standards, and applicable regulations.
  • Participate in supervision, quality assurance activities, program reviews, and required meetings.

Professional Development & Program Support

  • Participate in required training and professional development related to substance use, recovery coaching, Motivational Interviewing, family support, trauma-informed care, and other relevant practices.
  • Maintain professional boundaries and demonstrate respect for participants’ individual recovery choices, cultures, identities, and lived experiences.
  • Participate in agency initiatives and special projects as assigned.
  • Maintain flexibility to provide services outside traditional business hours when necessary to meet participant and program needs.

 

Required Qualifications

  • High school diploma, GED, or HiSET.
  • Relevant experience in recovery support, case management, behavioral health, family support, human services, or a related field.
  • Knowledge of substance use disorders, co-occurring conditions, recovery support services, and multiple pathways of recovery.
  • Ability to establish supportive, professional relationships with parents and caregivers experiencing complex challenges.
  • Strong communication, documentation, organization, and time-management skills.
  • Ability to provide services independently in homes and community settings.
  • Bilingual (English/Spanish) required.
  • Valid Massachusetts driver’s license, reliable transportation, and appropriate automobile insurance.


Preferred Qualifications

  • Associate or bachelor’s degree in Human Services, Social Work, Psychology, Behavioral Health, or a related field.
  • Certified Addiction Recovery Coach (CARC) or other recognized Recovery Coach credential, or willingness/eligibility to obtain certification.
  • Experience providing case management, recovery coaching, or family support services.
  • Experience working with DCF-involved families.
  • Training in Motivational Interviewing, trauma-informed care, substance use disorders, or co-occurring disorders.
  • Experience with Virtual Gateway/OASIS or similar human services data systems.
  • Lived or living experience that contributes to an understanding of recovery, when appropriate and relevant to the position.

Work Environment & Physical Requirements

  • Services are provided in participant homes, community locations, treatment and healthcare settings, and NNCC offices.
  • Regular local travel is required, and transportation of participants may be required when authorized.
  • The position requires flexibility to accommodate participant needs, including occasional early morning, evening, or other nontraditional hours.
  • Duties may require sitting, standing, walking, bending, and carrying program materials weighing up to 25 pounds.
