ESSENTIALFUNCTIONS:
- Conductcomprehensive assessments to individuals by assessing their medical history,motion of movements, and evaluating physical limitations to diagnose movementdysfunctions
- Developthe appropriate therapy treatment interventions based on the needs of theindividual, in addition report progress of long- and -short term treatmentgoals
- Restoresindividuals' function, alleviating plain, and administrate medically prescribedphysical therapy treatment
- Evaluatescurrent mobility, strength and diagnosis of individuals to determine if therapytreatment interventions need to be modified
- Assistindividuals with therapy by administering manual exercises, giving massages andusing therapeutic equipment as necessary
- Providessupport, counsel, advice, training and education for individuals, family,caregivers and other professionals on the importance of being consistent withtherapy treatment plans
- Efficientlyassembles extensive documentation on individuals' progress, care services andtherapy adjustments, when necessary, that aligns with standard medicaldocumentation and facility protocol/procedures documentation
- Collaborativelywork with other healthcare professionals to provide comprehensive care
- Mustdisplay excellent active listening skills, critical thinking skills,verbal/written communication skills, time management skills
- Mustdisplay compassion, integrity and respect to all individuals
- Supportingadditional responsibilities and/or tasks as needed for the department
PHYSICAL REQUIREMENTS:
- Physically,be able to lift, transfer and assist patients while providing therapyinterventions during physical activity
- Must beable to lift and carry items up to 25lbs, including assisting individuals whenneeded
- Abilityto visually be able to perform job duties, including examining individuals andreading information from printed sources and computer screens
OTHER REQUIREMENTS:
- Education: Must be a graduate of an accredited Physical Therapy program
- Licenses/Certifications: Must hold an active and in good standing Wisconsin Physical Therapist License and CPR Certification
- Follows/adheres to organization, federal, and state regulations and guidelines, including Resident/Client rights
- Maintains neat, well-groomed appearance, adhering to dress code of organization/program
