Job description


This position is responsible for the day-to-day operations of the case managers, social workers, utilization review staff, and case management assistants, under the general direction of the Case Management Director. The position will integrate knowledge, skills, and experiences to meet the needs of patients and families throughout the continuum of care. They rely on extensive experience and judgment to accomplish responsibilities. A wide degree of creativity and latitude is expected in order to lead and direct the work of others.
Responsibilities
  1. Assist the director of the department with support of clinical and administrative case management and social service roles through participation, problem solving and scheduling of staff in order to facilitate appropriate patient flow and services.
    1. Plans and updates in-service education programs for all case management staff annually.
    2. Conducts case review and high-risk screening evaluations weekly with staff.
    3. Performs weekly quality reviews and communicates findings to both staff and management team.
    4. Assist director in selection of team members.
    5. Provides for departmental orientation, onboarding, and initial competency checks for all new team members.
  2. Provides case management services to assigned population in addition to coordinating staff assignments in order to assure appropriate levels of service.
    1. Assures adequate case management coverage to meet customer expectations.
    2. Performs routine case management duties including planning, assessment, resource allocation and outcome tracking.
    3. Plans for staffing variances as evidenced by adequate coverage for all areas.
  3. Provides up to date clinical expertise to case management staff including new interventions and therapies, changes in clinical protocols and updates in clinical case management services in order to meet the needs of all assigned patient populations.
    1. Monthly education provided to staff regarding emerging trends in healthcare services.
    2. Secures specific case management educational materials and distributes on a weekly basis to staff.
    3. Conduct annual mandatory training to all staff including fire, infectious disease, disaster preparedness.
  4. Reports specific outcome measures including productivity, referral patterns, cost savings, utilization in order to trend and provide appropriate analysis of service delivery.
    1. Provides scheduled reports on specific criteria-based measures to Director.
    2. Provides analysis of data related to outcomes.
    3. Makes changes to service and service delivery based on outcome data analysis to improve service levels.
  5. OTHER DUTIES AS ASSIGNED
    1. Consistently performs 12 organizational Service Standards focused on Values, Service and Quality.

Qualifications
Experience-Five year's experience in case management in a healthcare setting. Some management experience.
Education-RN with BS/BSN or MSW
Training - Case management training.
Special Skills - Excellent computer skills, skilled in interpersonal & human relations; oral & written communication; problem solving & critical thinking; time management/prioritization; and proficiency with hospital software programs
