Job description

The Social Worker is responsible for providing direct services to patients and their families. This includes counseling and seeking resources to answer unmet needs, as well as providing consultation services and in-service education programs related to the psychosocial aspects of health care to patients and their families.

  1. Bachelors Degree in social work or related field, Masters Degree preferred
  2. Licensed in the state as a RSW, GSW, MSW preferred
  3. Three to five years experience as a Social Worker or discharge planner in healthcare, LTACH preferred
  4. Basic knowledge of computer skills – Microsoft Office (Word, Excel, Powerpoint)
  5. Must be able to read, write, and speak English
