Job Description

Position includes being responsible for the completion of psychosocial assessments, adhering to facility admission process and providing for each residents social, emotional and psychological needs.  Actively participate in interdisciplinarycare plan meetings.  Document progress notes that relate to residents care plan.  Assist the resident with alternative housing as necessary. This is a full-time position.

Qualifications
  • Bachelor's Degree in Social Work or related field.
  • licensed Social Worker in the State of MN
  • health care experience preferred
  • must meet and state and/or Federal requirements for the position
  • basic understanding of computer technology
  • must have ability to read, write and follow oral and written directions at a level necessary to accomplish the job
  • must have knowledge of State and Federal Long-Term Care guidelines
  • Must have demonstrated skills in communication, documentation, assessment and social work methods techniques.
  • must maintain regular attendance
