Job description

Requirements/Qualifications

•Master Level or registered in the state of practice as required by state regulations 

• BLS/CPR by American Heart Association

•CPI Certification (Crisis Prevention Intervention –Blue Card)

•1+ years previous experience working in Psychiatry Hospital setting and/or substance abuse

•Bilingual (English/Spanish) (Preferred)

Responsibilities

•In this role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional 

•Advanced self-management, planning and organization skills with the ability to work independently with minimal supervision

•The Social Worker assists in to resolving the social and psychological problems 

•Empowers patients to be proactive with regard to their health and personal well-being

•The Social Worker provides clinical coordination and therapeutic interventions through assessment, referral and interdisciplinary planning for high risk populations

•The Social Worker also expedites linking patients with systems in the community to provide them with resources, services and opportunities to achieve and maintain maximum functioning, and conduct
