Job Expectations:

Assist in planning, organizing, implementing, evaluating and directing the overall operation of the Social Service Department. The Social Worker will provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental and psychosocial well-being.

Job Responsibilities:

  • Resident/Family/Physician Intervention
  • Psychosocial Services
  • Vision, Hearing, Dental Services
  • Assessments & Care Planning
  • Discharge Planning

Requirements:

  • Prefer 2 Years Social Work experience in a Long Term Care or Hospital
  • 4 year Degree
  • Licensed Social Worker in the State of Texas

Education / Qualifications:

· 4 year Degree

· Social Worker License

Knowledge / Experience:

· At least 2 years experience in a Skilled Nursing Facility or Hospital

Licensure / Certifications:

· Social Worker License
