Salary: $65,000-$75,000 per year (DOE)

What You'll Do
  • Coordinate and oversee the facility's social services program
  • Complete psychosocial assessments and develop individualized care plans
  • Assist residents and families with discharge planning and community resources
  • Advocate for resident rights and promote quality of life
  • Provide emotional support and crisis intervention for residents and families
  • Work closely with the interdisciplinary team to ensure residents receive comprehensive care
  • Maintain accurate documentation in accordance with state and federal regulations
  • Help ensure compliance with all applicable healthcare regulations
Qualifications
  • Bachelor's degree in Social Work or a related human services field preferred
