Reports directly to the Social Services Manager

As a Social Services Coordinator at The Springs Health Center, you'll play an essential role in supporting residents and their families through life's transitions, helping ensure every resident receives the emotional, social, and psychological support they need to thrive.

As our Social Services Coordinator, you will:

  • Build meaningful relationships with residents and their families
  • Assess residents' social, emotional, psychological, and adjustment needs
  • Develop and coordinate individualized care plans
  • Provide support and guidance during challenging life transitions
  • Assist residents and families with discharge planning and community resources
  • Support residents experiencing dementia, behavioral health concerns, grief, or loss
  • Collaborate with nursing, therapy, physicians, and interdisciplinary team members to ensure quality resident outcomes
  • Maintain accurate documentation and ensure compliance with federal, state, and community regulations
  • Advocate for residents while promoting dignity, independence, and quality of life

Here are a few of the qualifications we need you to have:

  • Bachelor's Degree in Social Services or related field required; Master's degree preferred.
  • A minimum of three (3) years' social service experience within a skilled nursing facility preferred.
  • Equivalent combination of education and direct experience may be considered
  • Knowledge of Federal and State regulations for skilled nursing facilities
  • Strong communication skills and a heart for service
  • Dependable, compassionate, and calm under pressure
