Social Worker – Discharge Planner

Facility: Nassau Rehabilitation and Nursing Center
Location: Hempstead, NY
Schedule: Full-Time | Monday–Friday | 9:00 AM–5:00 PM
Position Type: Short-Term Unit
Salary: $75,000–$80,000 annually

Join Our Team as a Social Worker – Discharge Planner

Nassau Rehabilitation and Nursing Center is seeking a compassionate and experienced Social Worker – Discharge Planner for a full-time, short-term position. The ideal candidate will have a strong background in discharge planning and healthcare, with experience helping residents and families navigate successful transitions of care.

Key Responsibilities
  • Coordinate and manage the discharge planning process for residents

  • Conduct psychosocial assessments and interviews with residents and families

  • Develop individualized discharge plans based on each resident’s needs and goals

  • Coordinate with residents, families, physicians, nursing staff, and interdisciplinary team members

  • Arrange appropriate post-discharge services, equipment, transportation, and community resources

  • Communicate with hospitals, home health agencies, rehabilitation providers, and other healthcare organizations to facilitate safe transitions

  • Provide education and support to residents and families regarding discharge plans and available resources

  • Participate in care plan meetings and interdisciplinary team discussions

  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements

  • Advocate for residents while promoting independence, dignity, and continuity of care

Qualifications
  • Minimum of 1–2 years of experience in discharge planning, preferably in a skilled nursing facility or healthcare setting

  • 2 years of Social Worker experience in an SNF preferred

  • Bachelor’s Degree in Social Work or a related field required

  • Experience with Sigma EMR preferred

  • Strong communication, organizational, and interpersonal skills

  • Excellent knowledge of discharge planning and community resources

  • Compassionate, resident-centered approach

  • Ability to work effectively with residents, families, and multidisciplinary healthcare teams
