In this role,you'llplay a vital part in delivering high-quality psychosocial care for patients receiving cancer treatment across our regional sites. A caregiver in this position works days,8:00AM to5:00PM.


A caregiver who excels in this role will:

  • Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional,financialand environmental problems.

  • Perform psychosocial assessments and provide interventions toassistpatients and families. This may include counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problemsolvingand coordinating family/team meetings.

  • Provide and/or document treatment recommendations, professional recommendations, care coordinationinterventionsand treatment plans.

  • Engage patients and families in adhering to the treatment plan by reducing oreliminatingbarriers.

  • Maintain a network of resources for consultation and connect patients with community resources.

  • Supervise social work interns andassistwith training new caregivers.

  • Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.

Minimum qualifications for the ideal future caregiver include:

  • Master's Degree in Social Work from a CSWE accredited program

  • State Licensed Social Worker (LSW)

  • Two years of experience in a hospital or healthcare setting

  • Licensed Independent Social Worker (LISW) within five years of hire

Physical Requirements:

  • Manual dexterity to operate office equipment.
  • May require periods of sitting, standing and the ability to walk to various locations throughout the Foundation to attend meetings.
  • Must have normal or correction vision
  • Must be able to clearly communicate verbally by phone.

Personal Protective Equipment:

  • Follows Standard Precautions using personal protective equipment as required.

Pay Range

Minimum Annual Salary: $57,510.00

Maximum Annual Salary: $87,697.50

The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate's actual compensation will be determined after taking factors into consideration such as the candidate's work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.).
