JOB SUMMARY
Assists in meeting the psychosocial needs of residents in a long-term care facility. Collaborates with the inter-disciplinary team to develop and carry out individualized care plans to enable each resident to achieve their optimal level of health and well-being and maximize their quality of life.
QUALIFICTIONS
  • MA LCSW required.
  • A minimum of 2-3 years' experience in a long-term care setting.
  • Solid working knowledge of discharge planning, Medicare/Medicaid, OBRA regulations, community resources, social work methods and techniques.
  • Excellent communication, interpersonal, and computer skills required.
  • Knowledge of electronic medical records, MDS, and Care Plans.
