About the Role

Usingownautomobile, provides patient and family assistanceafterhoursand weekends. Performs direct and indirect total nursing care of the Hospice patient and familyin accordance withthe St. Francis Hospice Standards for Nursing Practice in the home setting; instructs family in simple nursing proceduresin accordance with the Hospice Interdisciplinary Plan of Care.

  • Provides care andmaintainsawareness of total nursing needs of patients/families assigned to him/her andis responsible forseeingtheyare fulfilled.
  • Triage hospice patient calls telephonically and makes on-call focus visits as needed for pain and symptom management and pronouncements, according to Federal, JCAHO, andappropriate Standardsof Nursing Practice as defined by the Hospice Program.
  • Notifies on-call physician on an as needed basisregardingpatient's condition, changes in the patient's plan of care, med orders, and pronouncements.
  • Monitors and documents patient's reaction to symptom management and pain control.
  • Reports all medication errors, adverse drug reactions, fall, and any unusual occurrences by completing an Incident Form and documenting in Adverse Events in All scripts.
  • Arranges for requisition of equipment needed in home setting whenappropriate, such as oxygen and/ orsuction equipment.
  • Delivers/picks up Hospice medications, and supplies in the home setting whenappropriate.
  • Sends tasks messages to Interdisciplinary team members, including social services, pastoral/spiritual care, and volunteer services when needed.
  • Sends Primary Case RNs all tasks of triage calls relating to their patientsin order toenable them to follow up andidentifyproblems for evaluation and to coordinate the plan of care to best meet the patient/families needs.
  • Responsible for initiating telephone calls to patients, if requested by Case RN, for afterhours follow up and/or visit.
  • Properlydisposesof narcotic meds during pronouncementvisit s.
  • Consults with Patient/Family Care Coordinator - Home Care on problems/safety concernsencounteredin caring forpatient/family.
  • Assists in providing anoptimalsafephysical and emotional environment for bothpatientand family.
  • Teaches,supervisesand counsels the patient and family membersregardingthe nursing care needs and other related problems of the patient.
  • Chartsaccurate,currentand complete clinical notes and assessments on all triage patients.
  • Adheres to and effectively communicates Hospice's philosophy, goals, andobjectivesto community health agencies and to the public.
  • Assists in the referral, intake and discharge process after hours as necessary, such as completing referral forms and faxing to call center, writing out med orders and faxing to MD for SOC RNs in the field after hours, and obtaining revocation/discharge signatures as needed.
  • Assists in orientation and development ofnew staffmembers in the Hospice Program.
  • Is responsible formaintainingtheappropriate insuranceverification on file with the business office as well assubmitscompleted mileage voucherina timelybasis.
  • Will work in a clinical role ason-call nurse on scheduled weekends and holidays according to rotation process.
  • Will work in a clinical role ason-call nurse after working hours on weekdays due to emergency or staff shortage.
  • Ensuresthat all patient information is held in strict confidence.
  • Works together with hospice inpatient RN when a patient is transferred into the facility for GIP management.
  • Ensuresthat when charting, only approved abbreviations found in the procedure manual are used.
  • Performs other related duties as assigned.
