Per-Visit Rates:
$75 per regular PT Visit
$112.50 per PT Eval
$187.50 per PT SOC

Job Description:
Physical Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.
  • Evaluate patients and perform appropriate therapy interventions
  • Develop a comprehensive plan of care and coordinate multi-disciplinary care
  • Perform prompt clinical documentation and visit notes
  • Continued communication with physician, care team members, patient and family
  • Comfortable working in all aspects of the home health setting
  • Implement and maintain physical therapy protocols
  • Manage patient treatment and outcomes directly, or through PTA supervision
Requirements:
  • Current Texas Physical Therapist (PT) license
  • One year of physical therapy experience
  • Experience with Homecare Homebase is helpful, but not required
  • Previous Home Health experience is helpful, but not required
  • Current Texas driver's license
  • CPR certification
