What are soap notes?

SOAP notes are a standardized method used by healthcare professionals to document patient encounters. The acronym stands for Subjective, Objective, Assessment, and Plan, representing the four sections of the note. This structure helps ensure that all relevant information about a patient's condition, the provider's assessment, and the planned treatment is clearly recorded and communicated. SOAP notes are widely used in medical, dental, and mental health settings to maintain accurate and consistent patient records.

What are the key skills and qualifications needed to thrive in writing SOAP Notes, and why are they important?

To excel in writing SOAP Notes, professionals need a solid understanding of clinical documentation, patient assessment, and relevant medical terminology, often supported by healthcare training or licensure. Familiarity with electronic health record (EHR) systems and documentation standards is essential for accurate and efficient record-keeping. Attention to detail, strong organizational skills, and clear written communication help ensure thorough and effective patient records. These skills are crucial for maintaining legal compliance, supporting continuity of care, and facilitating effective healthcare delivery.

What are some common challenges in accurately documenting SOAP notes, and how can these be overcome?

One of the most common challenges in documenting SOAP notes is ensuring clarity and consistency while capturing all relevant patient information within time constraints. It's important to distinguish between subjective and objective data, avoid ambiguous language, and document findings as concisely as possible. Utilizing electronic health record (EHR) templates, maintaining up-to-date clinical knowledge, and regularly reviewing best practices can help improve the quality and efficiency of SOAP note documentation. Collaboration with colleagues and ongoing training can also support accuracy and compliance with legal and institutional standards.

Job description

Massage Therapist โ€“ Medical Massage Apprenticeship
Clinical Massage Therapy (CMT)

Who We Are
Clinical Massage Therapy (CMT) is a leader in the growing medical massage field. We work alongside medical providers to help clients manage pain, recover from injury, and improve function โ€” and we believe skilled clinical therapists are developed through mentorship, education, and practice.
Our team is made up of intuitive, curious massage therapists who want to deepen their understanding of the body and learn how to work clinically. We are constantly learning: taking continuing education courses, participating in peer-to-peer reviews, refining hands-on skills, and improving our ability to write clear, medical-grade treatment notes.
If youโ€™re interested in medical massage but donโ€™t yet feel โ€œexperienced enough,โ€ this role was designed for you.
About the Role
We are seeking a licensed Massage Therapist to join our clinic through our Medical Massage Apprenticeship Program. This position is ideal for therapists who want to transition into medical massage and are motivated to learn, grow, and work through challenges with support.
You will learn how to:
  • Treat a clientโ€™s chief complaint using clinical reasoning
  • Work with physician referrals and post-injury or post-surgical cases
  • Integrate deep tissue, myofascial release, and trigger point therapy safely and effectively
  • Communicate clearly with clients about treatment goals and anatomy
  • ย Document care using supportive, easy-to-learn SOAP notes that help track progress and tell the story of each clientโ€™s healing.
No prior medical massage experience is required. A willingness to learn, receive feedback, and grow is essential.

What Youโ€™ll Do
  • Provide customized massage therapy sessions based on a clientโ€™s chief complaint and/or physician prescription
  • Integrate deep tissue, trigger point therapy, and myofascial release as appropriate
  • Educate clients on treatment-phase care vs. maintenance-phase care
  • Maintain accurate and timely SOAP notes (training provided)
  • Build strong therapeutic relationships with clients
  • Work collaboratively with a supportive clinical team
  • Maintain a clean, safe, and professional treatment environment
  • Manage time effectively, starting and ending sessions as scheduled
What Weโ€™re Looking For
  • Current state massage therapy license and liability insurance
  • Interest in medical or clinical massage and a desire to continue learning
  • ย Ability or willingness to develop confidence working with muscular attachments and insertions in clinically relevant areas (hip, pelvic, inner thigh, sacral, sternal or pectoral), supported by mentorship and clear clinical protocols

  • Willingness to confidently and professionally explain why certain areas are treated when addressing conditions such as low back, hip, or knee pain
  • Strong verbal and written communication skills
  • Professionalism, integrity, and a high ethical standard
  • Compassion and a genuine desire to help people manage acute or chronic pain
Experience with medical massage techniques is helpful but not required. Mentorship and training are provided.

Schedule
Part-Time Positions Available

  • Approximately 25โ€“28 hours per week
Why Clinical Massage Therapy?
If youโ€™ve ever thought, โ€œIโ€™m interested in medical massage, but I donโ€™t feel ready yet,โ€ youโ€™re exactly who we want to talk to. Weโ€™re looking for therapists who are intuitive, motivated, and willing to grow into excellent clinicians with guidance and support.
If youโ€™re ready to learn, weโ€™re ready to teach.
