• State or federal-issued photo ID must be presented at the facility and worn as a photo badge at all times.

• Uniform required per  site policy: light grey scrub top with matching scrub bottoms or black slacks.

• Float assignment: Mon / Tue / Wed at Starkville Manor (Starkville, MS); Thu / Fri float to (Winona, MS). Reliable transportation between sites is required.

Responsibilities

  • Evaluate and treat patients in a post-acute/SNF setting, developing evidence-based PT treatment plans.
  • Deliver skilled PT interventions targeting mobility, strength, balance, pain management, and functional independence.
  • Document evaluations, daily notes, progress notes, and discharge summaries within required timeframes.
  • Collaborate with OT, SLP, nursing, and the interdisciplinary team to coordinate resident-centred care.
  • Educate patients and families on home exercise programmes, fall prevention, and safe discharge planning.
  • Meet facility productivity standards while maintaining the highest standard of clinical care.

Requirements

  • Current, unencumbered Physical Therapist licence in Mississippi — required before start.
  • Graduate of an accredited Physical Therapy programme (DPT preferred).
  • Minimum 1 year of SNF or post-acute rehabilitation experience preferred.
  • Current BLS/CPR certification.
  • Strong documentation skills and ability to meet productivity targets.
  • Must be comfortable with a float assignment across two Powerback/Avardis sites.
  • Reliable transportation between Starkville and Winona, MS is required.
