Job description

Speech-Language Pathologist (SLP) | Full-time | Outpatient | 2131 College Ave Bluefield, VA 24605

H2 Health is seeking a Speech-Language Pathologist to join our outpatient team in Bluefield, Virginia. In this role, you'll work with a diverse outpatient population while providing individualized, evidence-based therapy services. You'll collaborate with Physical Therapists, Occupational Therapists, physicians, caregivers, and other healthcare professionals to help patients maximize communication, cognition, swallowing function, and independence.

Your Role
  • Perform comprehensive speech-language evaluations and develop individualized plans of care
  • Assess and treat communication, language, cognitive-linguistic, voice, fluency, and swallowing disorders
  • Deliver evidence-based interventions designed to improve communication skills, cognition, safety, and quality of life
  • Collaborate with patients, caregivers, physicians, and interdisciplinary team members to achieve optimal outcomes
  • Educate patients and families on treatment strategies and home programs
  • Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time
  • Utilize the Raintree EMR platform for documentation, scheduling, and patient management
Requirements
  • Master's degree in Speech-Language Pathology from an accredited program
  • Current Virginia Speech-Language Pathologist license or eligibility for licensure
  • Certificate of Clinical Competence (CCC-SLP) preferred or eligibility to complete Clinical Fellowship Year (CFY)
  • CPR certification or willingness to obtain as required
  • Strong communication and interpersonal skills
  • Commitment to providing high-quality, patient-centered care
  • New graduates and Clinical Fellows are encouraged to apply
