About the Job
@Home Therapy is a growing home healthcare company providing in-home Physical Therapy (PT), Occupational Therapy (OT), and Speech Therapy (SLP) services throughout New York. We are seeking an organized, detail-oriented Patient Care Coordinator / Scheduler to help manage patient intake, scheduling, insurance verification, and ongoing care coordination.
This position is ideal for candidates with experience in medical scheduling, patient coordination, healthcare administration, home health, medical office operations, case coordination, or patient intake.
Responsibilities:
  • Coordinate new patient referrals and admissions.
  • Verify insurance eligibility and benefits.
  • Schedule and assign PT, OT, and SLP clinicians based on geography, availability, and patient needs.
  • Serve as the primary point of contact for patients, caregivers, therapists, and referral sources.
  • Manage patient intake, onboarding, and follow-up communications.
  • Track visits, authorizations, documentation, and scheduling changes within the EMR system.
  • Maintain accurate patient records and data entry.
  • Handle inbound calls, patient inquiries, and scheduling requests.
  • Communicate with therapists regarding schedules, documentation, and patient status updates.
  • Support day-to-day operations to ensure timely and efficient patient care delivery.
Qualifications:
  • 1+ year of experience in a medical office, healthcare administration, home health agency, scheduling role, or patient coordination position.
  • Strong organizational and multitasking abilities.
  • Excellent phone, communication, and customer service skills.
  • Comfortable working with computers, EMR systems, Microsoft Office, and data entry.
  • Ability to prioritize tasks in a fast-paced healthcare environment.
  • Detail-oriented, dependable, and professional.
Preferred Qualifications:
  • Experience with home health, outpatient therapy, rehabilitation services, healthcare scheduling, or patient intake.
  • Familiarity with insurance verification and authorizations.
  • Experience using EMR or healthcare software systems.
