Job description

Apollo Home Healthcare is seeking a Speech–Language Pathologist (SLP) to provide in-home speech therapy visits across Contra Costa County, Alameda County, and the Tri-Valley area. This is a W-2 per diem role ideal for SLPs looking for schedule flexibility, autonomy, and meaningful patient relationships. You’ll provide high-quality, one-on-one care to patients in their homes while collaborating with a supportive interdisciplinary team.

What You’ll Do

As a Home Health SLP, you will:

  • Perform speech-language and swallowing evaluations in patients’ homes
  • Develop and implement individualized Plans of Care
  • Provide skilled speech therapy interventions
  • Recommend communication devices and adaptive equipment as needed
  • Assess home environments for safety and functional communication needs
  • Collaborate with physicians, nurses, PT/OT, and care teams
  • Participate in case conferences as appropriate
  • Complete timely, compliant clinical documentation
  • Educate patients and caregivers on exercises and compensatory strategies
  • Supervise home health aides when applicable to the plan of care
Qualifications
  • Active California Speech–Language Pathologist license
  • Master’s degree in Speech-Language Pathology
  • Current CPR certification
  • Home health experience preferred (but not required)
  • Strong documentation and communication skills
  • Ability to work independently in the field
  • Reliable transportation and valid driver’s license
