Schedule:

Sundays: 12:00 pm - 4:00 pm

Mondays: 8:30 am - 5:00 pm

One (1) Saturday per month: 8:30 am - 5:00 pm

Additional shifts may be required occasionally to provide coverage as needed.

Summary:

The Activities Assistant is responsible for assisting the Activities Manager in planning, organizing and directing the overall operation of the Activities Department in accordance with The Glenridge on Palmer Ranch (GPR) established policies and procedures, as well as federal, state and local standards, guidelines and regulations. In accordance with a Member's comprehensive assessment and under the direction of the Activities Manager, the Activities Assistant is responsible for ensuring that the activities program is designed and conducted in such as manner to include the interests, physical, mental, spiritual and psychosocial well-being of each Member.

Responsibilities:

  1. Assists the Activities Manager with the development of and implementation of Member care plans and comprehensive assessments as assigned in the Carrol Center, Highlands and/or Isle of Skye
  2. Conducts activity functions and/or programs as assigned
  3. Ensures a schedule of activities are prepared and distributed via calendar
  4. Communicates to Activities Manager recommendations for adapting the activities to the needs and desires of the Members
  5. Assists with supervision and organization of all off campus activities
  6. Assists with the coordination of basic transportation services for activities as needed
  7. Assists with assessments, quarterly notes, and pictures upon admission of a Member to the Carroll Center, Highlands and Isle of Skye and assists with the creation of meaningful activities.
  8. Attends Monthly review of Members by providing activity perspective of care planning as requested
  9. Ensures activities are performed within the departmental policies and procedures established
  10. Maintains safe equipment and environment, which ensures Members and staff are safe
  11. Ensures participation of other departments involved with activities is coordinated
  12. Ensures Activity orientation interview is performed with every Member within 30 days admission.
  13. Maintains files with care plan activity objectives on each Member
  14. Maintains confidentiality of members' PHI (Protected Health Information) and medical records information as well

Qualifications:

  • Must have at least a High School diploma or equivalent. At least 1 year of experience in a long-term care setting (skilled nursing, assisted living or memory care) preferred.
  • Proficiency in understanding of computer software as it relates to the job.
  • Experience with electronic medical records preferred
  • Excellent decision-making and analytical skills
  • Excellent communication skills and problem-solving skills
  • Excellent organizational and time management skills
  • Must beempathetic and have an affinity for working with a diverse senior population
