About the Role:

A Passionate Helping Hand Home Health Care LLC is looking for a dedicated and compassionate Physical Therapist to join our growing team in Indianapolis, IN. In this rewarding role, you'll make a real difference by delivering personalized, in-home physical therapy services that help patients regain independence and improve their quality of life. If you're passionate about patient-centered care and love building meaningful connections, we want to hear from you!

Responsibilities:

Conduct comprehensive in-home patient evaluations to assess functional abilities and develop individualized treatment plans

Provide hands-on physical therapy treatments including therapeutic exercises, manual therapy, and mobility training

Monitor and document patient progress, adjusting care plans as needed to meet evolving goals

Educate patients and family members on home exercise programs, fall prevention, and safe mobility techniques

Collaborate with interdisciplinary care teams including physicians, nurses, and occupational therapists

Maintain accurate and timely clinical documentation in compliance with Medicare and state regulations

Coordinate discharge planning and recommend appropriate follow-up care or community resources

Requirements:

Active Physical Therapist (PT) license in the state of Indiana

Graduate degree (DPT, MPT, or equivalent) from an accredited physical therapy program

Home health or outpatient clinical experience preferred

Strong knowledge of Medicare home health guidelines and documentation standards

Valid driver's license and reliable transportation for home visits throughout Indianapolis

Excellent interpersonal and communication skills with a patient-first mindset

Ability to work independently and manage a flexible caseload
