Job description

POSITION: Registered Nurse DEPARTMENT: Medical

STATUS: Non-exempt/hourly

REPORTS TO: Site Director

LOCATION: Hopkinsville, KY

POSITION OVERVIEW

The Registered Nurse (RN) works directly with the physicians and health care team to provide optimal care for LIVWELL CHS clients. The RN also works directly with clients and their families. The RN performs frequent patient evaluations including monitoring and tracking vital signs, administering medications and vaccines, etc. The RN is usually the first to notice problems or raise concerns to the physicians. Because the RN has frequent direct contact with LIVWELL CHS clients, they are vital in helping patients deal with their illness.

JOB RESPONSIBILITIES
  • Review and triage/treat labs daily on patients; document all treatment and phone calls in EMR
  • Provide HIV education regarding prevention, chronic disease, treatment, and adherence.
  • Refer patients appropriately to outside providers, emergency rooms, or walkin clinics; retrieves records from outside sources; user defines; outside labs/testing and update future encounters
  • Check triage line multiple times daily and refer patients to providers who are requesting medications or to be seen
  • Order, maintain, and provide HIV tests for the clinic; perform controls on all new lot numbers/inventory
  • Order and maintain inventory of clinic medications, medical supplies, vaccines, hygiene products, and adherence tools
  • Run reports to include: Kasper, vaccine, new patients, genotypes, drug interaction, and referrals
  • Initiate and complete chart reviews for upcoming encounters, including recent labs, referrals, screenings, vaccines, and phone calls
  • Call/eprescribe medications ordered by physicians including running Kasper reports on all controlled substances and initiate/complete prior authorizations as needed
  • Attend all required meetings
  • Work closely with colleagues for patient assistance, transportation, mental health, and dietary needs
  • Report all new cases to: STD surveillance, HIV/AIDS state database, an Pregnancy Registry
  • See walkin patients for nurse visits including: oPCN shots
  • BIZ injections oFlu vaccines
  • Dressing changes oEvaluation of symptoms and referral to appropriate provider/level of care
  • Room patients to include: ovital signs
  • chief complaint oupdate clinical history
  • new patient interviewing omedication review
  • assist providers with GU exam, PAP, breast exams, and various other procedures/treatments
  • Work in conjunction with inhouse pharmacy to monitor patient assistance with copays, adherence to medicine regimens and blister packs
  • Complete “to dos” in a timely manner, ensuring all aspects of treatment plan is conducted
  • Assist Clinic Manager in updating/creating Clinical Policies and Procedures as needed
  • Review outside records and new patient enrollments as needed to assist providers
  • User define vaccines, labs, genotypes, etc. as needed
  • Educate and link patients to LIVWELL CHS’s Part B, C, and housing programs or other outside services. Utilize a variety of strategies such as motivational interviewing
  • Assist with retention efforts by contacting patients that have rescheduled, cancelled or no shown two times in a row to reengage into care.
  • Follow up with new/returning patients no later than three days following their initial/return appointment.
  • Follow up with patients who have been newly prescribed antiretroviral therapy medications or had a change in medication regimen within one week of initiation/change.
  • Monitor patient for a total of four visits to track and monitor progress and efficacy of the care plan. Evaluate the care plan at each of these visits and adapt changes as needed.
  • Ensure patient privacy and confidentiality in all phases and activities.
  • Maintain knowledge of HIV/AIDS care, funding sources such as Ryan White Programs and services through selfstudy and training.
  • Maintain a positive work atmosphere by acting and communicating in a manner so that you get along with patients, coworkers and managers.
  • Assists in establishing continuous quality improvement that supports the accomplishment of the organization’s mission.
  • Performs other duties as directed/logically associated with the job
  • QUALFICATIONSEDUCATION/EXPERIENCE:
  • Associate Degree in Applied Science of Nursing
  • Registered Nurse (RN)
  • Licensed as required by state regulations Kentucky & Illinois
