Pay: $38.00/hour
New Hire Sign-on Bonus: $5,000.00
Schedule: Monday 8am to 4pm, Tuesday 8am to 4pm, Wednesday 8am to 4pm, Thursday 8am to 4pm, Friday 8am to 4pm

What You'll Do:
  • Provide community-based nursing care coordination across CFCM, Shared Living (SLA), and BHDDH programs
  • Conduct comprehensive health assessments and develop individualized nursing care plans
  • Coordinate care across medical, behavioral health, specialty providers, hospitals, and community services
  • Serve as a clinical resource to Shared Living Providers, offering guidance, training, and ongoing support
  • Monitor chronic conditions, medications, treatment plans, and health outcomes
  • Review hospital discharges, ER visits, and clinical recommendations to ensure timely follow-up
  • Deliver education on health promotion, medication safety, infection control, and chronic disease management
  • Ensure compliance with BHDDH, EOHHS, Medicaid, HIPAA, and agency standards
  • Participate in interdisciplinary team meetings, quality reviews, and incident management processes
  • Advocate for individuals served and promote person-centered, community-based care
