Description:
Shalom Hospice is seeking a dedicated and experienced Nurse Practitioner (NP) to join our GIP/Palliative Care team in Nashville, TN, working exclusively within the HCA hospital systems. This is a full-time salaried role focused solely on providing inpatient care to patients in General Inpatient (GIP) and palliative settings—no home hospice visits required.

The NP will serve as the clinical lead for symptom management, palliative consults, and family support, ensuring excellence in serious illness care for patients receiving hospice-level care in an acute setting.

Key Responsibilities:

  • Provide direct inpatient care and daily rounding for hospice GIP and palliative patients

  • Conduct comprehensive assessments, manage symptoms, and adjust treatment plans as needed

  • Collaborate with hospitalists, specialists, case managers, and Shalom Hospice’s IDG

  • Lead and document goals-of-care conversations with patients and families

  • Ensure smooth transitions of care, coordinating with discharge planners and hospice leadership

Qualifications:

  • Active Nurse Practitioner license in Tennessee with prescriptive authority

  • Board Certification in Adult-Gerontology, Family Practice, or Palliative Care

  • Minimum 2 years of inpatient palliative or hospice experience, preferably in a hospital setting

  • Strong knowledge of hospice eligibility, GIP regulations, and Medicare documentation

  • Outstanding communication skills and a commitment to compassionate, interdisciplinary care
