Home Health Occupational Therapist (OT)

Per Diem | San Mateo County — Dedicated Territory Assigned

We are currently looking for a compassionate, organized, and team-oriented individual to fill the role of Occupational Therapist (OT) position for the SF - Peninsula area.

We are hiring Occupational Therapists for Peninsula teams. You will be assigned to one dedicated territory — San Mateo County — so you can build familiarity with your patients and your area without being stretched across both.

Your Role

  • Provide skilled occupational therapy services focused on ADLs, adaptive equipment, and functional independence
  • Develop, implement, and manage individualized plans of care based on physician orders
  • Report changes in patient condition and collaborate with physicians, case managers, and the Director of Nursing
  • Maintain accurate, timely clinical records using AI-supported documentation tools

Requirements

  • Current, active California Occupational Therapist license (required)
  • Minimum of 1 year of home health experience strongly preferred
  • Valid California driver's license
  • Strong organizational skills, clinical judgment, and a compassionate communication style
