Alliance Human Services, Inc. is seeing a dedicated Intake Coordinator to join our not-for-profit, treatment foster care agency. The Intake coordinator plays a critical al role in ensuring sooth referrals, placements, and service coordination for children in foster care. If you are detail-oriented, able to make clinically sound decisions, and committed to the well-being of children and families, we want to hear from you!
What You'll Do:
โ€ข Accept and manage all referrals for placement, ensuring completion of all required intake documentation.
โ€ข Participate in the orientation, training, evaluation, and assessment of potential foster family home applicants.
โ€ข Establish and maintain a referral matching system, including tracking upcoming discharges within 30, 60, and 90 days.
โ€ข Ensure all documentation is uploaded and updated in our electronic records system, Child Trax, and meets corporate, licensing, and regulatory standards.
โ€ข Evaluate and assess children for placement and continued placement, ensuring necessary services are established based on their needs and permanency plan.
โ€ข Conduct home visits and foster parent support meetings as needed, including evening and weekend hours.
โ€ข Provide pre-service and in-service training for foster parents, including weekend sessions.
โ€ข Participate in the rotational on-call system for emergency and crisis support.
โ€ข Prepare court reports, attend judicial placement reviews, and represent the agency professionally.
โ€ข Assist with legal, licensing, regulatory, and certification activities.
โ€ข Attend vendor fairs, community events, and foster parent recruitment activities to promote Alliance Human Services.
โ€ข Offer ideas and suggestions to improve quality and increase visibility in the state.
โ€ข Maintain current CPR/First Aid certification.
โ€ข Other duties as assigned.
What You Need:
โ€ข Bachelor's Degree (Master's preferred) in Social Work, Human Services, or a related field.
โ€ข At least two years of experience in foster care, social services, or case management.
โ€ข Strong problem-solving, decision-making skills, and organizational skills.
โ€ข Advanced writing skills are critical for documentation, reports, and communication.
โ€ข Ability to work flexible hours, including evenings and weekends as needed.
โ€ข Valid driver's license and reliable transportation.
โ€ข Current CPR/First Aid certification or willingness to obtain.
