Position Overview

The Vision Works Youth Counseling Center is seeking a dedicated Licensed Independent Clinical Social Worker (LICSW) to join our team on a full-time basis. You will assess and address the psychosocial needs of clients, facilitating their emotional growth and empowering them to navigate life's challenges.

Work Environment

This position is fully remote, allowing you to work from the comfort of your home while engaging with clients through virtual sessions. You will have the flexibility to create a work schedule that meets the needs of the clients while ensuring your availability for on-call duties. Key Responsibilities
  • Conduct in-depth assessments of clients' psychosocial needs, integrating emotional, mental, and social factors into personalized care plans.
  • Collaborate with clients and their families to develop effective strategies that align with their goals, ensuring tailored interventions that promote healing.
  • Provide counseling and emotional support to seniors facing life transitions, such as bereavement or health changes, guiding them through each step with empathy and expertise.
  • Advocate for clients' rights to access community resources, assisting in securing necessary services and benefits for their well-being.
  • Engage with interdisciplinary teams composed of healthcare professionals, caregivers, and family members to enhance a cohesive approach to client care.
  • Regularly visit clients (virtually) to monitor well-being, assess living conditions, and address any emerging concerns regarding their safety and health.
  • Facilitate family communication to resolve conflicts and ensure that everyone's needs are acknowledged and met during the care process.
  • Offer educational resources regarding available community programs, support groups, and other services that can enhance clients' social engagement.
  • Maintain accurate documentation of client assessments, care plans, and progress notes, ensuring compliance with regulatory standards and organizational policies.
  • Stay up-to-date with best practices in social work and related regulations to continually improve service delivery.
Patient Population / Client Focus

The role primarily focuses on seniors and their families, assisting them in managing various emotional and psychological challenges. You will address issues such as anxiety, depression, grief, and the complexities associated with aging. Your work will significantly impact our clients' quality of life, providing them with tools and resources to regain control over their emotional health.

Qualifications
  • Master's degree in Social Work from an accredited institution is mandatory.
  • Current and valid state licensure as a Social Worker (LICSW).
  • A minimum of two years of professional experience in social work, ideally within a community center or geriatric care environment.
  • Profound understanding of social work principles, aging-related issues, and community resource availability.
  • Exceptional assessment, counseling, and problem-solving skills.
  • Ability to empathize and build rapport with clients, particularly seniors and their families.

Preferred Experience
